the only thing not working on this mockup is the "Other" textbox, ie - if the student selects "other" a text box should appear (maxlength=35).

21	When we print out the PDFlib files, I will want an abbreviated name for each individual Activity. For example, Instead of printing out the three levels of pulldowns "In-School Club > Laguage Club > French" , I will simple want to print "French Club"	21	Not Started

23	As per my last email, I am attaching a list of names for each individual extracurricular activity that I'd like to use to describe them on the Application PDF. In creating this list, I realized I was shortsighted in a few of the classifications on the list that I sent you last. Specifically, there were 4 items that were combined but need to be separated out into their own classification (i.e. - they should be 2 separate things). 26	have uploaded a close-to-final version of the 2008 CM application on quest4. (/usr/home/qbridge/2008 CM App.pdf) I believe all the block names are correct, but was unable to test it.
